What Is Doxycycline And How Does It Work?

Doxycycline is an antibiotic which works against a wide range of bacteria (broad spectrum). It belongs to the tetracycline class of antibiotics, and is fat soluble, which helps it penetrate skin tissues and hair follicles.

It targets acne through two main pathways:

  • Antibacterial action. Stops the growth of Cutibacterium acnes bacteria in blocked hair follicles.

  • Anti-inflammatory action. Stops inflammatory substances from causing the redness, swelling, and pain associated with papules, pustules and nodules.

What Acne Types Does Doxycycline Help With?

Doxycycline is prescribed for moderate to severe inflammatory acne, especially when breakouts cover larger areas, or are difficult to manage using only creams/gels.

Acne Type

Appearance

Does Doxycycline Help?

Papules & Pustules

Inflamed, red spots or pus-filled pimples on the face, chest, or back.

Yes. Highly effective at reducing bacteria and inflammation.

Widespread Body Acne

Moderate to severe acne across shoulders, back, or chest.

Yes. Oral treatment covers the whole body systemically.

Comedones (Blackheads)

Non-inflammatory clogged pores.

Partially. Best when combined with a topical retinoid, (E.g., Differin or Epiduo)

Deep Nodules & Cysts

Severe, painful, hard lumps under the skin.

Limited. Severe nodulocystic acne usually requires specialist dermatologist care.

Does Doxycycline Work For Acne Scars?

Doxycycline can help with concurrent acne flare-ups and can help fade flat red or dark marks left by acne (hyperpigmentation), but it will not fix deep, pitted or raised acne scars. These deep scars involve permanent tissue loss in deeper skin layers.

How Effective Is Doxycycline For Acne Treatment?

Clinical trial data confirms doxycycline significantly reduces inflammatory lesions:

  • Targeted reduction of bacteria. Doxycycline targets bacteria under the skin surface quickly, though visible changes may take several weeks.

  • Purging phase. Some patients may notice a temporary “purge” during the first 2 to 4 weeks, with breakouts, as the skin clears debris.

  • Combined effectiveness. Combining doxycycline with a non-antibiotic gel or cream leads to the best results, and helps prevent treatment resistance.

Doxycycline works gradually over time. Remaining consistent in dosing is crucial to success. Maximum clinical benefits are evaluated between 8 and 12 weeks.

How Long Does Doxycycline Take To Work For Acne?

  • Weeks 1 to 4. This is when the body adapts to treatment, and some may experience “purging” as bacteria levels drop.

  • Weeks 4 to 8. Inflammation calms, and new spot formation visibly slows down.

  • Weeks 8 to 12. Maximum benefit is seen, with clearance of red papules and pustules and smoother skin texture.

If there has been little or no improvement after 12 weeks of treatment, contact the Quick Meds team to evaluate your treatment plan and explore alternative treatments.

How To Take Doxycycline For Acne?

Take doxycycline exactly as prescribed by your clinical team.

  1. Dose. Take one 100mg capsule once daily, at the same time everyday.

  2. Administration. Swallow the capsule whole, with a full glass of water whilst standing or sitting upright. Do not lie down for at least 30 minutes after taking it, to prevent throat irritation.

  3. Stomach Irritation. If you experience stomach irritation, take the capsule with food or milk.

  4. Timing gap for minerals. Leave a 2-hour gap between doxycycline and antacids or mineral supplements (containing iron, calcium, or magnesium) as they can affect doxycycline absorption.

What If I Miss A Dose?

If you forget a dose, take it as soon as you remember, unless it is almost time for your next dose.

If it is nearly time, skip the missed dose and take the next dose at the next scheduled time. Do not double dose to make up for a missed dose.

Taking doxycycline alongside other acne treatments

Doxycycline is usually taken alongside a non-antibiotic cream or gel, such as Skinoren, or Differin, for maximum results whilst reducing the risk of antibiotic resistance.

You should not take doxycycline alongside:

  • Other oral antibiotics

  • Oral retinoids, such as isotretinoin.

When prescribed for acne, doxycycline is typically used with topical medications like benzoyl peroxide or retinoid gels. This combination helps improve results, targeting acne from different angles. Benzoyl peroxide can reduce bacteria, retinoids can help clear pores and doxycycline can control inflammation.

Storage recommendations

  • Avoid storage with extreme temperatures or humid environments.

  • Store below 25°C in its original packaging.

  • Keep out of reach and sight of children.

Side Effects Of Doxycycline

As with all medicines, doxycycline can cause side effects, though not everyone experiences them.

Frequency

Side Effects

Management Tip

Common (Up to 1 in 10)

Nausea, throat irritation, headache,  photosensitivity.

Take with a full glass of water whilst sitting upright. Use SPF 30+ when outdoors.

Uncommon (Up to 1 in 100)

Dizziness, mild skin rash, upset stomach

Take with food or milk if stomach upset occurs.

Severe reactions

You should stop taking doxycycline and seek immediate medical attention, by calling 999 or visiting A&E if you experience:

  • A severe allergic reaction (anaphylaxis). Swelling of the face, lips, tongue, or throat, or difficulty breathing.

  • Severe skin reactions. Widespread skin blistering, peeling, or severe rash (Stevens-Johnson syndrome).

  • Antibiotic-associated colitis. Persistent, severe, or bloody diarrhoea, with abdominal cramps.

  • Intracranial hypertension. Severe, persistent headache accompanied by visual disturbances.

Who Is Doxycycline Suitable For?

Doxycycline is licensed for use in adults and children over 12 years with moderate to severe acne.

At Quick Meds, we supply treatment only to patients above 18 years of age. For patients under 18, you must speak to your doctor.

Who Shouldn’t Take Doxycycline?

Do not take doxycycline if you:

  • Are allergic to doxycycline, other tetracycline or any of the inactive ingredients within the capsules.

  • Are pregnant, planning a pregnancy or breastfeeding. (can affect bone and tooth development in infants)

  • Are under 12 years old. (risk of permanent tooth discoloration)

  • Suffer from severe kidney or liver impairment.

  • Have systemic lupus erythematosus, or a history of severe bowel inflammation.

Interactions And Precautions

If you are taking other medicines, including herbal or over the counter-remedies, make sure you inform your prescriber during the online consultation:

  • Antacids & indigestion remedies. Preparations containing aluminium, magnesium, or calcium interfere with doxycycline absorption. Leave a 2 hour gap between them.

  • Oral retinoids (isotretinoin). The combination can increase the risk of intracranial hypertension. Strictly forbidden.

  • Blood thinners (warfarin). Doxycycline can enhance blood-thinning effects, increasing the risk of a bleed.

  • Contraceptive pill. If you experience severe vomiting or diarrhoea lasting over 24 hours whilst on doxycycline, follow your pill’s missed-pill guidelines.

  • Sun exposure & sunbeds. Doxycycline causes photosensitivity, making skin more likely to get sunburnt. Avoid sunbeds, wear protective clothing, and apply a high SPF sunscreen everyday.
