Orforglipron is a new medication recently gaining MHRA approval for its use in type 2 diabetes and weight management. As with most other weight loss medications, it is a GLP-1 agonist designed to help people lose weight and lower blood sugar levels.

Orforglipron, or the branded Foundayo is a new weight loss medication formulated as a once-daily tablet. Developed by Eli Lilly, it belongs to the GLP-1 class of medications.
Unlike earlier GLP-1 treatments formulated as peptides, which require strict administration methods, orforglipron is a small-molecule non-peptide compound. Because of this, the tablet does not need any strict dietary restrictions, and can be taken with or without food, at any time of day - offering ease of administration.
As a GLP-1 agonist, orforglipron activates the natural GLP-1 (glucagon-like peptide-1) hormone. It acts on receptors in the brain and digestive system to regulate hunger by:

Foundayo is not suitable for everyone. It should not be taken by:
Anyone with a personal or family history of Medullary Thyroid Carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN2)
Patients who are allergic to orforglipron or any of its ingredients.
Orforglipron is taken as a tablet, taken once everyday. Due to the way it is formulated as a small-molecule, it offers greater flexibility over other oral GLP-1 treatments, which require strict fasting rules.
The new orforglipron tablet:
Can be taken at any time of day.
Can be taken with or without food.
Can be taken with any liquid, without restriction on water volume.
Tablets must be swallowed whole, do not cut, crush, chew or disperse them.
Patients new to treatment must start at the initiation dose of 0.8mg once daily. This is taken for 30 days before increasing further. This process allows the body to adjust to treatment safely, minimising the risk of side effects.

In the ATTAIN-1 phase 3 trial, over a 72-week period:
Participants taking the highest orforglipron dose achieved an average weight loss of 12.4% compared to 0.9% in the placebo group.
60% of participants lost 10% of their initial body weight, whilst 40% of participants lost 15%
