What Is Soolantra and How Does It Work?

Soolantra is a white/light yellow cream containing 10mg of ivermectin per gram (1% w/w). It is used to reduce inflammatory spots, papules, and pustules associated with papulopustular rosacea in adults.

Ivermectin works in two ways to tackle rosacea-affected skin:

  1. Anti-inflammatory action. It blocks the action of cytokines - a chemical that leads to inflammation. This reduces welling, redness and papules from forming.

  2. Antiparasitic action. It kills Demodex skin mites. Demodex mites harmlessly live in hair follicles, but an overgrowth can lead to rosacea.

Soolantra should be applied thinly once daily over the entire face, using a pea-sized amount for each of the five facial regions (forehead, chin, nose, and both cheeks). It should not be applied to the eyes, lips, or mucous membranes.

What Dose Of Soolantra Will I Need?

Soolantra cream is only available in a single strength: 1% w/w

The cream is applied thinly, no more than once daily. Applying more cream, or more frequently will not speed up results or improve its effectiveness but rather will increase the risk of skin irritation, burning and dryness.

How to Use Soolantra: Dosage and Administration

To apply Soolantra cream, follow the below step-by-step instructions:

  1. Cleanse - wash your face with water and a mild, soap-free cleanser and pat completely dry.

  2. Measure - squeeze a pea-sized amount of cream onto your fingertip for each of the 5 facial regions.

  3. Apply - spread thinly and evenly across your face, avoiding the eyes, lips, nostrils and mouth.

  4. Clean hands - wash hands thoroughly with soap and water immediately after use.

  5. Drying time - allow the cream to dry completely before applying moisturisers, sunscreen or cosmetics.

Soolantra should strictly be applied no more than once daily at bedtime.

If You Miss an Application

If you forget to apply Soolantra, skip the missed application and apply your next dose the next evening.

Never apply a double dose or apply extra cream, to make up for a missed dose.

Storage Recommendations

  • Use within 6 months of first opening.

How Long Does Soolantra Take to Work?

Most patients notice an initial improvement, with reduction in spots and inflammation within 2 to 4 weeks of consistent use. Maximum clinical benefits are usually achieved after 12 to 16 weeks of continuous treatment.

Rosacea is a chronic condition and improvement can take time. Keep using Soolantra as prescribed unless you develop a significant reaction or your prescriber advises you to stop.

Some people may experience a temporary worsening of rosacea during the first week of treatment. Known as initial purging, this flare-up is an inflammatory response to the dead mites - this resolves with continued use.

Who Is Soolantra Suitable For?

Soolantra is suitable for adults aged 18 and over who have been diagnosed with mild-to-moderate inflammatory papulopustular rosacea.

Cautions and Precautions

If you have any of the following conditions, let your prescriber know:

  • Severe liver disease.

  • Highly reactive, highly sensitive skin or a history of severe contact dermatitis.

  • Use of medicines that interact with ivermectin (e.g., ketoconazole, clarithromycin).

Who Shouldn’t use Soolantra

Whilst Soolantra can safely be used by most, you should not use Soolantra cream if you:

  • Are allergic to ivermectin or any of the inactive ingredients in the cream (e.g., parabens, cetyl alcohol, stearyl alcohol, propylene glycol).

  • Are pregnant or planning a pregnancy.

  • Are breastfeeding

  • Are under 18 years of age.

Side Effects of Soolantra

Soolantra is generally well tolerated, but like all medicines, it can cause side effects.

Common Side Effects

The most common side effects, affecting up to 1 in 10 people include:

  • Skin burning or stinging sensation on the skin.

  • Mild skin dryness, flaking or itching.

  • Local skin irritation or redness (erythema)

Less Common/ Rare Side Effects

Less common side effects include:

  • Allergic contact dermatitis

  • Worsening of rosacea flare-ups (temporary purging)

  • Severe eye irritation, if cream enters the eye.

If you experience any of the following symptoms, wash your face immediately, stop using the cream and seek immediate medical attention via NHS 111 or A&E:

  • Severe swelling

  • Painful blistering

  • Signs of an allergic reaction (swelling of the lips, tongue, mouth or throat, difficulty breathing, hives)

Interactions And Things To Avoid

Though very little of the drug actually enters the bloodstream, you should inform your prescriber of any other medication you may be taking. This includes medication that is prescribed, purchased over the counter, and herbal products.

Medicines That Interact with Soolantra

Soolantra (ivermectin) is mainly broken down in the liver by the CYP3A4 enzyme. Taking medications which inhibit CYP3A4 may increase ivermectin levels in the body by reducing its metabolism.

Tell your prescriber if you take:

  • Antifungal medicines - like ketoconazole or itraconazole.

  • Macrolide antibiotics - such as clarithromycin or erythromycin.

  • Protease inhibitors or antiviral drugs - commonly used for HIV.

If you are unsure if the mediation you take is a CYP3A4 inhibitor, let your prescriber know.
Do not stop any prescribed medicines or change their doses without medical advice.

Soolantra and Alcohol

There is no interaction between alcohol and Soolantra. However, alcohol itself can trigger flushing.

Minimising alcohol intake helps to optimise your treatment results.

Soolantra and Sun Exposure

UV radiation can damage the skin, triggering a rosacea flare-up. It is recommended to apply sunscreen (SPF 30+) daily over Soolantra once the cream has completely dried.

What Are The Alternatives To Soolantra?

If Soolantra is unsuitable for you, alternative treatments available from Quick Meds include:

  • Finacea 15% Gel - containing azelaic acid. A non-antibiotic treatment that unclogs pores, calms redness, and treats inflammatory bumps.

  • Metrogel 0.75% Gel - containing metronidazole. A well-established antibiotic gel used to reduce localised inflammatory spots and swelling.

The most appropriate option depends on your symptoms and individual circumstances, which you can discuss with your clinician. If you are unsure, feel free to contact the team before placing your order.
