Wegovy is a weight loss medicine, launched in the UK in September 2023 containing the active ingredient, semaglutide.
As a pre-filled pen containing 4 weekly doses, Wegovy is available in multiple dose strengths ranging from 0.25mg all the way up to 7.2mg. Which dose is right for you? Can you start at the higher dose, what determines a dose change?

Wegovy is available in the following dose strengths:
0.25 mg
0.5 mg

1.7 mg
2.4 mg
A higher maintenance dose of 7.2mg has recently been approved, as the next step up after the current maximum dose of 2.4mg.
At present, a single 7.2mg pen does not exist. For the higher dose, patients are required to adminster multiple injections of 2.4mg to make up the 7.2mg dose.
Wegovy is a relatively high-risk medication. Once injected, the drug lasts in the body for at least 5 days. To ensure the wrong dose does not remain in your body for this length of time, either being ineffective or too strong (causing side effects), its important to work with your provider for the most appropriate dose.
Starting at too high a dose can be a shock to the body. It can increase the risk of side effects, particularly if you've never used the treatment before. In line with the licensed dose, Wegovy must be initiated at the starting dose of 0.25mg.
When continuing with treatment, consider increasing the dose only if there is a clinical need to. Here at Quick Meds, we encourage patients to maintain at the lowest, most effective dose as long as possible. Increasing the dose whilst you are achieving desired results on your current dose will not make the treatment more effective. It may however, increase the results of side effects.
Additionally, whilst higher doses may increase the rate of weight loss, note that rapid weight loss carries risks in itself, such as gallstones and pancreatitis.
The risk of side effects must be weighed up against getting the full benefits of the drug. If you are acheiving slower results, or notice increased hunger and appetite, it may be time to increase to the next dose.
Remember, when increasing, this must be done sequentially. I.e you cannot jump from 0.25mg to 1mg, and must use 0.5mg for a minimum of 4 weeks first.
So long as a patient meets the criteria upon treatment initiation, an individuals body weight does not generally influence the dose.
To be eligible for treatments, patients must have a:
BMI of 30 or above
BMI of 27 or above, with a weight-related health condition such as high blood pressure or type 2 diabetes.
If, during the course of the treatment a patient has successfully attained their goal weight, and are now within a healthy BMI range, your prescriber may suggest reducing the dose to prevent the risk of getting underweight.
Whilst other medications can affect Wegovy, usually the other medications are adjusted rather than changing the Wegovy dose.
When used alongside insulin or other blood sugar lowering drugs, Wegovy can lead to a dangerous drop in blood sugar levels (hypoglycemia). In this case, prescribers would often adjust the dose of insulin or diabetic medication rather than changing the Wegovy dose.
Due to its impact on delaying gastric emptying, Wegovy may affect the absorption of other oral drugs. This doesn't affect standard painkillers but may affect medications that require rapid absoprtion or have a narrow therapeutic window, such as:
Oral contraceptives - if Wegovy causes vomiting or severe diarrhoea, effectiveness of contraceptive pills may be reduces.
Some antibiotics
Blood thinners like warfarin - requiring more frequent monitoring
It's important to discuss any other medication you may be taking with your provider, to allow for an accurate check on drug interactions and whether your Wegovy dose needs amending.
All patients start treatment at 0.25mg administered once-weekly. This is continued for 4 weeks.
At this point, most patients will then increase to 0.5mg. Those who experienced side effects may need to stay at 0.25mg for an additional 4 weeks, to allow the body longer time to adjust.
Once at 0.5mg, the dose is adjusted in line with individual response. I.e. a dose which offers a balance of steady weight loss with minimal to no side effects.
For those unsure if they should stay at their current dose, increase or decrease, work with your provider for tailored advice based on your medical history and response to treatment so far.
This dose escalation schedule is guidance only - dose strengths should be tailored to the individual. Some people may require a longer time at the lowest dose if they struggle with side effects when starting the medication and it may take much longer to make the last dose increase up to the maintenance dosage.
Wegovy should be administered once-weekly. i.e. on the same day every week, at roughly the same time.
Changing this schedule can affect your results. Clinical data shows stretching the doses to every 10 or 14 days reduces treatment effectiveness.
If you miss a dose of Wegovy, it should be injected as soon as possible and within 5 days after the missed dose.
If more than 5 days have passed, skip the missed dose and the next dose should be taken on the regular scheduled day.
If you miss more than 2 consecutive doses, you may need to talk to a medical professional about how to safely restart
It's important to have regular medical checkups when taking any medication. But if you're experiencing significant side effects, including severe abdominal pain, changes in your heart rate, or signs of an allergic reaction, it's important to seek medical help immediately.
If you're worried you've taken too much Wegovy, you may experience low blood sugar, or severe nausea and vomiting. Again, seek medical help quickly.
It's important to never adjust your Wegovy dose yourself. Always speak to a doctor, pharmacist, or GP.
Check to ensure you have the correct pen that your healthcare provider prescribed
Check the expiry date on the back of the box to make sure the medication is still in date.
Wegovy can be administered into the thigh, upper arm or stomach. Choose your injection site, making sure the area is visibly clean.
Make sure to rotate injection sites every week, to avoid the risk of injection site reactions.
Inspect the Wegovy solution through the dose window. It should appear clear and colourless.
Air bubbles in the solution are normal, and will not affect the dose.
Remove the pen cap
Attach a new needle straight on and screw to tighten
Turn the dose knob until you see the flow check symbol (.._ ) - this is usually 2 clicks
Hold the pen with the needle pointing up, and remove the outer and inner needle covers.
Press and hold the dose button until the dose indicator lines up with 0mg
You should see a drop of Wegovy at the needle tip, this indicates the pen is ready for use.
If a drop does not appear, check the flow again.
Turn the dose selector all the way until it stops turning. You will see your dose line up with the dose pointer.
Insert the needle completely into the injection site at a 90 degree angle.
Press and hold down the dose button until it returns to 0.
Continue to hold the dose button for an additional 5 seconds before removing the needle.
You may see a drop of Wegovy at the tip of the needle, but this is normal and does not affect your dose.
If you notice a small amount of bleeding at the injection site, press lightly on it with some clean gauze.
Remove the needle and safely dispose in a sharps bin
Replace the pen cap and store appropriately until your next dose.
