Trimethoprim is an antibiotic designed to fight bacterial infections. Whilst it is commonly used to treat uncomplicated urinary tract infections (UTIs) and cystitis, it also plays a role in managing acne and chest infections.
To grow, bacteria need a specific nutrient called tetrahydrofolic acid. Trimethoprim blocks an enzyme that bacteria use to make this nutrient and in doing so, effectively ‘starves’ the bacteria, stopping them growing and multiplying.
As trimethoprim targets bacteria, it has no effect on infections caused by viruses, such as the flu or common cold.
If you are experiencing symptoms of a bladder infection, you can read our in-depth guide to cystitis and UTIs, to learn more about identifying symptoms and preventing future flare-ups.
When treating an uncomplicated UTI, the correct trimethoprim dosage is important to completely clear the infection.
For adult women, the standard trimethoprim dose is:
One trimethoprim 200mg tablet taken twice daily for 3 full days.
To get the most out of your treatment:
Space doses evenly. - Try to take the doses at the same time every day, roughly 12 hours apart.
Swallow whole. - With a whole glass of water. Do not crush, chew or break them.
Complete the full course. - Even if your symptoms completely disappear. Stopping early.
Read the PIL. - The patient information leaflet (PIL) contains comprehensive safety instructions, ingredient lists and warnings.
Patients frequently ask: is trimethoprim a strong antibiotic?
Rather than thinking about ‘strong’ or ‘weak’, clinicians evaluate antibiotics based on how sensitive they are against the bacteria you are targeting. Trimethoprim is very effective as it concentrates in the urinary tract, making it optimal for clearing the bacteria that reside here (E.Coli) during a cystitis breakout.
However, if you have a history of frequent infections, bacteria may have built up resistance to trimethoprim. In these cases a different antibiotic such as Nitrofurantoin can be used. This is a popular first-line alternative which works in a different way to trimethoprim.
Trimethoprim begins fighting the infection within a few hours of your first dose. Most patients notice a significant improvement in painful symptoms within 24 to 48 hours.
If symptoms do not start to improve after 48 hours of your first dose, or if they worsen significantly, contact your doctor immediately. Signs such as fever, chills, or back pain may indicate the infection is not responding to trimethoprim or is moving up to the kidneys.
Whilst generally very well tolerated, trimethoprim can cause side effects, though not everyone gets them.
The most common side effects, which are typically mild and go away on their own after finishing the course include:
Nausea or vomiting
Mild diarrhoea
Headaches
Mild skin itching.
For a comprehensive guide into managing these symptoms, please consult our detailed guide on trimethoprim side effects and safety.
If you experience signs of a severe allergic reaction (anaphylaxis), you should stop taking the medication and seek immediate medical attention. Signs to look out for include:
Swelling of the face, tongue, lips or throat.
Severe difficulty breathing or swallowing.
A severe, blistering rash.
Whilst generally a well tolerated medication, trimethoprim should not be taken by anyone who:
Has ever had an allergic reaction to trimethoprim.
Is pregnant, or planning a pregnancy.
Has severe kidney or liver disease.
Has blood disorders such as anaemia or porphyria.

Everything you need to know
Trimethoprim is used for bacterial infections, including UTIs and cystitis. It is also occasionally used to treat acne and chest infections.
Yes, you can drink alcohol whilst taking trimethoprim. There is no known interaction, however, alcohol dehydrates you and irritates the bladder, worsening UTI symptoms. It’s best to avoid alcohol whilst recovering.
Trimethoprim tablets can be taken either with or without food. If however, you find the antibiotics are making you feel nauseous, taking your dose with a small meal can help.
If you miss a dose, take it as soon as you remember - unless it is nearly time for your next dose. Do not take two doses together to make up for a missed dose.
Trimethoprim is effective for many uncomplicated UTIs. Rather than labelled as ‘strong ', it is specialised for urinary tract infections, being highly sensitive against the bacteria that cause symptoms.
